JOINING THE MILLGATE HEALTHCARE PARTNERSHIP

Accountable / Named GP for All Patients

All patients registered at the Surgery have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

Do you live within our Registering Boundary Area?

How to register

We are currently registering new patients. A patient may register by attending the surgery and handing in their medical card or by completing the appropriate form, which will be supplied on request. New patients must reside within the practice boundary area. We look forward to seeing you.

Disabled Patient Facilities

Disabled patient facilities are also available at this practice.
(Car park space, ramp, toilet etc).
